lshal: clean up ListCommand::fetchBinderized

Refactor fetchBinderized. Move logic for each TableEntry to
a separate fetchBinderizedEntry function.

* Change allDebugInfos to allTableEntries to contain more information.
* Use getPidInfoCached instead of allPids for cache.
* TableEntry can be default constructed.
