Set '%c' in LLVM_PROFILE_FILE for continuous mode 

Bug: http://b/194128476

If CLANG_COVERAGE_CONTINUOUS_MODE is set, enable continuous mode by
adding '%c' to LLVM_PROFILE_FILE.

Test: CLANG_COVERAGE_CONTINUOUS_MODE m and verify continuous mode works.
