libdm: refactor DM_TABLE_LOAD error

Now, the table string is formatted as:

  dm.cpp:332 DM_TABLE_LOAD failed:
  name=com.android.apex.test_pack_0.apex, table=linear 0 2305
  /dev/block/sda34 45588480: Invalid argument

, which is much easier to read.
