Floss: rework media profile management policy

This CL introduces the following changes:

* Initiate connection to missing profiles 6s after the first connection.
* Disconnect the device after 10s if profiles are incomplete (excluding
  AVRCP)
* Initiate connection to AVRCP only when A2DP is connected
* Initiate disconnection to connected profiles when one is disconnected
* Initiate disconnection to A2DP only when HFP is disconnected
* Initiate disconnection to AVRCP only when A2DP is disconnected
