floss: Update hcidoc to capture RnR collisions

Capture a few types of collisions from serializable commands:
* RnR
* Inquiry
* Connection

Only Inquiry is checked for a self-collision (haven't seen self
collisions for RnR or Connection in real usage) but we could update to
catch all collisions in the future.
