floss: Properly clean up ACL on disconnect

In order to clean up ACL on disconnect, we need to call the lowest level
disconnect functions in |classic_impl| and |le_impl| because that's
where the round-robin scheduler is configured. This fixes an issue where
classic reconnects weren't properly priming the round robin scheduler.
