AVRCP Controller during phone call

When starting a phone call from the AVRCP controller there is a loss of
audio focus before starting the call.  Some phones don't gracefully
accept pause and play commands while in a phone call or immediately
after.  Change the behavior to skip pausing audio from a phone that is
streaming audio since it's own telecom services should handle it.
