SAP: Fix following issues in SAP code

- Update connection state to upper layers only when SAP is connected i.e.
  status indication is sent to SAP Client.
- Show the notification on UI to disconnect from SAP Server only when SAP
  is successfully connected, and not when SapServer object is created.
- Add logic to handle ACL_DISCONNECTED to restart socketListener to
  handle scenarios where socket listener was not getting started due to
  abnormal ACL disconnect.
- Handle disconnect request during call ongoing state properly.
