Move "Report" to CallDetailActivity.

- Delete old report button/handling code.
+ Convert menu click listeners into single onMenuItemClick method.
+ Add and populate objectId on PhoneCallDetails, to be able to
tell from the CallDetailActivity whether to enable reporting.
