Finally fix our search to behave as expected

- Updated our order by to prioritize phoneType and contact id
- Remove the duplicate skipping so we can eliminate an extra query
- Create our where string at runtime to decide which items we need
to display and which should be excluded. If no mimetype is given,
we drop our provider's entries.
