PhoneBook: Specify file path when reading EFs of PBR

Some cards,like UICC with USIM application, have the same file ID for
phonebook EFs (3GPP TS 31.102, 4.4.2). For example,
EF_ANR/EF_ADN/EF_EMAIL id is "4FXX" under DFphonebook, but EF_IMG id
is "4F20" under DFgraphics and EFSAI is "4F30" under DFSoLSA. The path
may get mapped to unexpected EF resulting in incorrect behavior.

To avoid this, specify the EF Id and the path to uniquely identify
phonebook EFs when reading contacts.
