[DeviceAwareAppOp] Add device id as a new dimension to DiscreteOp 

In order to persist device attributed AppOp accesses in
DiscreteRegistry, we need to add device Id as a new dimension in the
memory object DiscreteOp. Also pass device Id to various methods in
DiscreteRegistry.
