frameworks/base - CDMA settings additions

These changes are the frameworks/base portion of CDMA UI changes.

Five settings are added:
   - In-call DTMF type (burst or long)
   - Emergency tone type
   - CDMA auto-retry
   - Hearing Aid Compatibility (on/off)
   - TTY mode
