Optimize for single provider / cred type case on the primary screen

1. Single provider: remove provider name from primary-page entries when they are from the same provider, since the provider icon + name will be shown on the top of the bottom sheet

2. Single cred type: when all primary-page entries are passwords / passkeys, then update the title to be "Choose your saved password ..." / "Choose your saved passkey ..." and remove the credential type from each entry.
