frameworks/base: unlink death notifications of AudioFocus clients 

Death notifications of AudioFocus clients isn't needed when
focus is abandoned or client is unregistered. The accumulated
death notifications causes excessive JNI references and causes
dalvik (hosting system_server) to abort the same.
