Extract TetherInterfaceSM to its own class.

Attempt to keep all existing logic in place, except:

 + Marked a constructor as public, rather than default visible.
 + Added TAG, DBG, VDBG, and decoder ringer statics.
 + Moved static constants related to USB IPs into TetherInterfaceSM.
