audio_policy: handle in call sonification for attached outputs of duplicating output 

When phone state enters IN_CALL with duplicating output in playback,
stream RINGTONE on deep buffer output will be muted once, when
duplicating output stopped, it will only decrease mute count for
duplicating output, RINGTONE on deep buffer output can't be unmuted,
then when BT headset disconnected, ringtone of incoming phone call
will be muted.
So when duplicating output stopped, we should handle in call sonification
for the attached outputs.
