Default subId and Dummy subId related fixes

- Fix to send ACTION_DEFAULT_SUBSCRIPTION_CHANGED intent,
  if a new SIM inserted while current default sub
  set to a DUMMY subId.
- Add support to always return first phone values for API's
  getDeviceId() and getDeviceSvn() of IPhoneSubInfo.aidl.
- Change few SubscriptionController member as protected.
