SubscriptionInfo not loaded correctly for dual sim

- code cleanup: replace deprecated getSubId api with
  getSubIdUsingPhoneId
- fix the bug in handleSimLoaded. we will only
  write subscriptionInfo of the lastest loaded sim, leaving incorrect
  parameter for the other sims in db.
