UiccController logging updates for bug reports.

Add more traces in the local log so that it's included as part of
UiccController#dump:
- Log slots' status in onGetSlotStatusDone
- Log updates in onMultiSimConfigChanged
- Log requests switchSlots
- Include phoneId in onSimRefresh log

Log human-readable event names instead of plain handler ints.

Also log in Rlog whenever we log to the LocalLog.
