SIM state can be changed back to LOCKED from LOADED

IccRecords and its derived classes do not give enough consideration
regarding the situation that the SIM state is changed back to LOCKED
from LOADED. It can happen if user attempts to change PIN1 by using MMI
code and fails until it is blocked. isLoaded(), getRecordsLoaded()
should not return true while the lock is effective.

Test: Confirmed the SIM state is changed to READY before LOADED.
