Telephony API extension v2

Added the following notifications:

public void notifyPreciseCallState(Phone sender);
public void notifyDisconnectCause(Connection.DisconnectCause cause, int preciseCause);
public void notifyPreciseDataConnectionFailed(Phone sender, String reason, String apnType, String apn, String failCause);
