audio policy: fix null ptr dereference in SwAudioOutputDescriptor::info() 

Commit 4b5c425a added SwAudioOutputDescriptor::info() method but the
implementation did not consider duplicating outputs with a null profile.

Bug: 285165253
Test: connect Bluetooth headsets with LDAC codec and play a ringtone.
