msm: clock-8084: Add clock entries for USB2 controller

Add the clock table entries that are used by the USB 2.0
(formerly "msm_otg") controller, along with the correct
device tree address-based device name. Also remove references
to "msm_ehci_host" as it not used on this SoC.
