usb: phy: qmp: Fix and update rev1 initialization parameters

This patch fixes one subtle but important bug: the offset
for QSERDES_COM_DIV_FRAC_START3 was incorrectly off by 8
bytes, causing PHY initialization to fail.

Also update the rest of the initialization parameters to
the required sequence for the rev1 PHY found on MSM8994.
