ARM: dts: msm: Update CPR device node for msm8909-PM8916

Use the per-fuse-corner initial voltage values as the ceiling
voltage values. This ensures that the closed-loop settled CPR
voltages can never exceed the fused open loop voltages even when
the device gets very hot. Add fuse bits used for reading CPR revision.

Also, enable the CPR controller to control the APC voltage.
