usb: dwc3-msm: Initialize HSPHY and SSPHY using QSCRATCH

Initialize SSPHY and HSPHY using qscratch registers. This
includes resetting SSPHY, enabling ref_clock for SSPHY and
configuring its parameters. As part of HSPHY initialization,
enable UTMI clock and clamp enable various HV interrupts. Also,
bypass VBUS filters required to detect VBUS change events.
