[PATCH] dvb: remove duplicate key definitions

The duplicate key definitions cause misinterpretations with other keys, e.g.
the "TELETEXT" key clashes with "CHANNEL_UP" and thus can not be used with
LIRC.  This patch removes these duplicates.
