msm: ipa: Add IPv6 NAT uc activation interface

Add IOCTLs for adding and deleting uc activation entries.
Amend Socksv5 add\delete entry algorithm to allow IPv6 NAT
and Socksv5 entries to co-exist in activation table.
Add debugfs entry to dump uc activation table.
