usb: phy: Add snapshot of QTI USB PHY drivers

This change adds USB HSUSB and USB QMP PHY drivers used
on QTI MSM platforms. This snapshot is taken as of msm-4.14
commit <a9cdbef8> ("Merge "usb: phy: Enable proper DP DM
masks for PHY interrupts"").
