iommu: msm: add snapshot of msm_iommu driver

This is a snapshot of the iommu v1 driver as of msm-3.10

msm: Remove mdmferrum changes")

In addition, drop older v0 support, hardware workarounds
which are no more needed while retain iommu_map_range,
iommu_unmap_range hooks to support legacy support.
