ARM: dts: msm: Add VADC nodes for msmtitanium

Add VADC and ADC_TM channel nodes to support clients to measure
voltage power and thermal clients to read system thermistors for
pmtitanium and include header which has the adc nodes defined
already for the PMIC 8950.
