soc: qcom: glink: Log subsystem and sequence number in SSR notifications

When sending an SSR notification, the G-Link SSR driver currently does
not log the name of the subsystem being restarted, nor does it log the
sequence number for the notification. This increases difficulty when
debugging.

Log the name of the subsystem being restarted and the sequence number of
the notification.
