msm: kgsl: Enable GPU per-process pagetables on 8996 v2

Enable GPU per-process pagetables on 8996 v2.

1) Use new CP packet 0x53 for GPU pagetable switch. This
new CP packet takes pagetable address and does IOMMU
pagetable switch and TLB flush atomically.
2) Fix a bug in cp_invalidate_state to use correct number
of ordinals.
