ARM: dts: qcom: Rename 8909w/8009w dts files

Renamed all 2100, 3100 and 2500 dts files with the chipset numbers
as prefix and removed some of the dts files from the Makefile which are
not needed anymore, this has done to reduce the confusion on dts files
for customers. For more clarity from boot logs, added chipset number
in model name for 2100,3100 and 2500 dts files
