ARM: dts: msm: Rework the MSM8992 freq plan

Due to the glitchy CDIV workaround, MSM8992 can no longer
support the 300MHz rate. Add the 384MHz and 480Mhz levels
back to the A57 plan; these levels will use the same voltage
vote as the 633.6Mhz level in order to be able to power
collapse at 600Mhz.
