usb: phy: Make phy drivers GKI compatible

This change starts using dwc3-msm.h header as all QTI PHY specific
changes are being moved from usb.h to dwc3-msm.h header. It also
removes below mentioned functionality:
- Performing USB only reset in USB + DP mode as hc died and restart
USB session functionality is now removed with dwc3 controller driver.
