drivers: lmh-dcvsh: Snapshot of LMH-DCVSh driver

This snapshot is taken as of msm-4.4 commit <db2c9496c893>
(drivers: lmh_dcvsh: Use allocated variable for LMH DCVSh sensor name).

Update the driver to use the thermal frameworks device tree based
registration for LMH DCVS sensor and cooling device. Also, add support
to place CPU frequency floor mitigation using LMH DCVSh.
