phy: usb: qmp: Update initialization sequence of USB QMP DP combo PHY

Power up the PHY before and after updating port_select and phy_mode.
This is required as updating port_select/phy_mode involves port reset
which power downs the phy. Also PHY global reset is not resetting USB
block in the phy. Hence add support for usb3 phy reset signal which
explicitly resets usb block in the qmp dp/usb combo phy. Remove the
port_select check so that phy is reset every time cable is connected.
