Clarify the information needed to create GitLab account

We have GitLab account creation blocked to prevent spamming and, we open account as per request to Helpdesk. The Information we need to create GitLab account can be asked more in details in this page.

What is the current behavior?
When a user wants to report an issue in GitLab, we ask the user to email Helpdesk in this doc. It says:
In case you run into such an error, please send a mail with the details of the ID you wish to create and the mail ID you got the error on, to helpdesk@murena.com
Which is not very clear as to what information are needed to be sent.
What is the improved behavior?
We can instead say this:
In case you run into such an error, please send a mail with the details of the ID you wish to create and the mail ID ie.
Full Name: Desired username: Email:
We can even add this to prefill an email for the user:
