tbs: Close Call Control Proxy on In Call Service clear

Call Control Proxy was not destroyed on Bluetooth Off. Not destroying
Call Control Proxy on Bluetooth off leads to unintended catch of
Bluetooth state change intents due to still registered listener in In
Call Service.
