Floss: Rework floss place active call

Floss previously placed active calls directly from a no-call state.
This worked due to Fluoride handling headset disconnects during
active calls by attempting to resume them.
Floss utilized this functionality incorrectly, but it functioned.

This change modifies the implementation of placing active calls.
It now involves dialing first and then answering the call to
ensure correct usage.
