RootCanal: Correctly handle HCI commands with invalid bd_addr or handle

Previously the command was at best ignored (bd_addr unknown) or
crashed RootCanal (connection handle unknown).
This change modifies the LMP implementation to send an HCI event
with status Invalid Parameters in both cases.
