gd: Patch HCI pdl definitions for ISO commands and events

- invalid op-code index for LE Set Cig Parameters Test
- in ISO H4 packets, iso_sdu_length is the total length of
  the sdu, not the length of the fragment
- rename peripheral_clock_accuracy -> worst_case_sca
