A2DP: Add logging when A2DP fails to open

* Make sure we output a log line whenever A2DP start request returns a
  failure state, namingly:
  - A2DP_CTRL_ACK_FAILURE
  - A2DP_CTRL_ACK_INCALL_FAILURE
  - A2DP_CTRL_ACK_DISCONNECT_IN_PROGRESS
