Give permission controller a version and icon

The icon is the same icon used to permission that do not specify their
own icon. It is a stylized phone with a "i" in it. The background is
gray as this app will have to integrate with all OSs and themes.

The version will be "API-version.update.bugfix". We leave space for 100
updates and bugfixes.
