Add “Rename device” on Bluetooth screen

* Add BluetoothDeviceRenamePreferenceController in bluetooth_screen.xml
* Refactor BluetoothDeviceRenamePreferenceController and BluetoothDeviceNamePreferenceController.
  Let these controllers can be init in xml.
