Revert^2 "Turn ContentProvider injection failures into errors."

This reverts commit 7637e39d.

Reason for revert: Restore this change.

This change is important and uncovers real problems.

I do not recommend reverting this again.

If tests start failing or devices start crashing, it is only because 
this change was _hiding_ those crashes before.
