ARM: dts: msm: Add support to read USB_IN_V for PM6150

Add calibration peripheral address needed in SW WA for
reading USB_IN_V channel. Add PMIC revid to skip WA for
version 2 and above of PM6150.

In addition, correct calibration setting for chg_temp.
