iommu: arm-smmu: Merge for msm-kona kernel upgrade 



Merge all iommu/smmu changes from msm-4.14 to msm-kona
as of:
'commit 72ac9228 ("iommu: arm-smmu: dump additional
smmu registers in fault handler")'.
